Update max supported resolution on MediaFoundationVideoEncodeAccelerator

Update max supported resolution on MediaFoundationVideoEncodeAccelerator

This CL adds new capabilities of MediaFoundationVideoEncodeAccelerator that was found
when testing on a new hardware -Intel HD P530-.
- Add 3840x2176 as the max supported resolution.
- Find input/output stream id using Get*putStreamInfo().
- Change CODECAPI_AVEncAdaptiveMode.
- Don't throttle output frame rate.
