This addresses some issues from CL 2116793002:

1. Name mangling isn't necessary, a private name in kernel contains a reference
to its library. So as long as we compute the correct library, mangling isn't
needed.

2. Name mangling isn't correct: only names used in mixin applications are
mangled, and that means that, for example, 'foo' in
test/kernel/regression/mixin_library.dart would be calling
PrivateName('_privateMethod', test.mixin_library), but class C in
test/kernel/regression/super_mixin.dart would define
PrivateName('test.mixin_library::_privateMethod', test.mixin_library).

In a later CL, I'll update kernel to print names differently so the library name
is included in the golden files.
