Can this flag change dynamically while the Document is around, or can it only be
set once, when the document is being loaded and CSP is parsed?  If the latter,
do we need this separate message, or could we just send the flag over as part of
DidCommitProvisionalLoad, similar to how we set the replicated origin today (in
NavigatorImpl::DidNavigate)?

It can change dynamically. (For example, if a <meta> element with a CSP gets
added via JS dynamically.)
