Shift polymer.html imports to custom elements.

Move the polymer.html into the individual custom elements (such as viewer-progress-bar) from index.html and index-material.html.

According to the Polymer docs, yes. I have never seen polymer.html imported
except when to create a new custom element. There is little use importing it in
index.html (and index-material.html).
