ContentHandler::OnConnect: replace the |url| param with |requestor_url|.

ContentHandler::OnConnect: replace the |url| param with |requestor_url|.

Darin had previously argued that we did not need |url|, as that can be
gotten from response. But we do need a way for content handlers to know
the url of the app connecting to them.
