Bernhard: please review.

The existing use of ProvisionalChangeToMainFrameUrl is silly for lots of
reasons. First, you don't care about all the random intermediate redirects that
happen, but only where you land as a result. So ProvisionalChangeToMainFrameUrl
is then equivalent to DidStartProvisionalLoadForFrame.

But that's silly too, because that's a subset of
DidCommitProvisionalLoadForFrame, which you're _already_ overriding.

So I consolidated and simplified everything.
