Moved modules files from bindings/core/v8/custom to bindings/modules/v8/custom.

Moved modules files from bindings/core/v8/custom to bindings/modules/v8/custom.

- V8CustomSQLStatementErrorCallback, V8EntryCustom and V8EntrySyncCustom should be placed under modules/v8/custom.
- Removed unused include (for modules).
