This change refactors the expression continuations. 
With the planned addition of statement continuations, I would prefer to use
short suffixes, such as: 
- EK instead of ExpressionContinuation
- SK instead of StatementContinuation
- A instead of Application or App

I like it that names in the code are now in sync with the names in the spec!
LGTM.

Originally I had doubts about two capital letters at the end of identifiers, but
it looks like Dart convention for identifiers permits two-letter capitalized
acronyms:

I think that having the names in sync makes it easier to review and/or modify
the specification and the implementation.
