I'm not completely happy with the "done headers" nomenclature (here and in the
boolean); it doesn't have high level semantic meaning to me.  Maybe replace it
with "failed/no match overwrite/replace"?  Just a thought; up to you.

I am inclined to keep it because it is in line with the various ways this state
machine is now demarcating headers vs response body phases. Other places where
we are using "headers" is STATE_FINISH_HEADERS* and DoneWithResponseHeaders()
