- add an integration test for execution.setSubscriptions (marked as failing)
- update the coverage.md format to allow marked covered tests as failing
- address two new deprecation warnings from the new version of package:test

I think we need to update the version ranges for the test package in the
pubspec.yaml files (in all four packages, just to be safe) to ensure that these
new methods are defined.

Thanks, I think I'll back those two changes (related to package:test) out of
this CL - they should have been separate anyway.
