There's a lack of context (well, other than the spec, but see my other comments)
for what the above are; a bit more of a comment about what they mean and what
the distinction between them is?

Hmm. origin is basically always guaranteed to be url.GetOrigin(). I'll comment
it for now. I'll file a spec issue to combine the fields. (Right now, the spec
theoretically allows someone to queue a report with an origin of https://foo/bar
and a URL of https://baz/quux.)
