FYI: I was able to add the field in the middle by adding ordinals to every
field:

[MinVersion=4] OnTaskCreated@4(int32 task_id@0,
                               string package_name@1,
                               [MinVersion=13] string? name@3,
                               string activity@2);

up to you if you want to keep this ordering or the one you preferred originally.
In any case, please add explicit ordinals to all parameters.
