Update generate_port_list.py to generate Markdown

Update generate_port_list.py to generate Markdown

Since we migrated from the wiki the docs/ directory now
contains markdown files.  The generated file also now used
URL references to avoid unreadable long lines.

This change updates the script and also includes the
regenerated list.
