Cleanup .gitignore file

Cleanup .gitignore file

Remove extensions that were the present for legacy gyp
generators such as make, vsms, and xcode.

Remove entries for legacy stuff that has since been removed.

Add a leading / for entries that apply to top level
items only.

Would you mind doing a double-check of thes extension wildcards I'm removing? 
Especially .gypcmd.. my understanding is that those now live in the gyp output
directory, is that right?

And I'm right in thinking that we no longer care about non-ninja?  (certianly
the SDK build doesn't care about non-ninja).
