Rename several classes in barback.

AssetProvider     -> PackageProvider
AssetGraphManager -> PackageGraph
AssetGraph        -> AssetCascade

Can you rename the "asset_graph" directory in test too?

Otherwise LGTM++++. The more I see it, the more I like "cascade".
