Just following up from the IRC comments..

Rather than calling make_pair to generate a new instance and then invoking the
copy-ctor, just use

QueryIconsSizeMap::value_type kQueryIconSizeData[] = {
  { "small", IconLoader::SMALL },
  { "normal", IconLoader::NORMAL },
  { "large", IconLoader::Large}
};

Same with 125-161
