I think what would be ideal here is a comment describing the full dataflow for
this class.  Maybe something at the top:
* When used to create a history entry, all fields except for db_handle are input
fields.
* When used to update a history entry, db_handle is used to identify the entry
to update, and fields ... are updated (or all fields except ... are updated).
* When used to query the database for existing items, all fields are returned.
